Chili Cook Off Entry Form

Please fill out this form to reserve your spot in the Chili Cook Off (spots are capped at 25 entries).
Funds raised from the Chili Cook Off will go to the Brenda Glass Multipurpose Trauma Center.
There are two options for payment to keep your spot, pay $10 now through Credit Card or select pay later and drop off $10 to the church office or give through ACH on the church website by clicking "Click to Give Online" and selecting "Give to Missions/Outreach" and putting "Chili Cook Off" in the comments.
Registration must be paid by March 12 or your spot will become available for an additional registrant.
